Long Term Services and Supports Pre-Screen
This pre-screen tool can help you find services and supports that you may be eligible for to meet a full range of needs over an extended period of time, including everything from meal preparation to housing options to personal care from a professional caregiver. You can start the process here to see if you might qualify for assistance. We welcome you or anyone helping you – a family member or advocate – to explore these options, now and in the future.
Completing the Pre-Screen
This will take you about 20-25 minutes to complete. It will ask you to tell us about yourself, your background, and what assistance you may need. When you are done, it will tell you what programs you may be eligible for that best serve your needs.
